The concrete company may be worth a try, call and see if they can explain why their mix wasn’t consistent. I use to do concrete work and also worked for a concrete dealer/manufacturer.

Zep degreaser from Lowes. With a wire brush and elbow grease. Ours was that way too. Fixed it right up.

how long ago ? are you watering your concrete ? don't do anything but add water daily to help the cure twice a day if you can. concrete is porus, I believe vinegar will cause major discoloration

you have moisture under your concrete. concrete hydrates vertically. If moisture mitigation was in your contract then you can hold the contractor responsible, if not then its just concrete being concrete.
