My theory is valve adjustment too tight on that specific valve, thus valve not cooling when seating. This was a common problem on class 8 diesel engines. Coming from a retired life long diesel mechanic.

When I worked for Caterpillar we would look at those broken valve stems under a microscope and preform a failure analysis on the fracture.

Only ever seen that in a John Deere 9.0, in a crusher that runs at max RPM . someone fitted injectors 10 days earlier but didn’t test all of them….
