How can we submit a formal report to this police department? It seems that the officer who just arrived walked straight in and tried to reach for the suspect's gun, despite the suspect being in an unstable state. He could have potentially de-escalated and prevented his partner from getting shot. He could have cost his partner’s life easily

Just FYI, the gun of the suspect got jammed after the first shot, the other gunshots you hear are from the cop that cover the other side of the vehicle and was able to take him down immediately
