i think the pills represent thanos to him and thats why he takes exra pills to ease the pain of losing the only thing that was helping him get through the games and converting the feeling he had for thanos whether it was live or not we dont know but we know feelings were there and when he died they transferred to the pill necklace and as soon as nam gyu finds out theres no pills left (theres no more left of thanos) he dies. you can see the look in his eyes when he realises and gives up. also as he uses more and more pills he gets more paranoid and crazy i think its both because the affects of the pills and the fact that with every pill he takes hes getting further away from thanos and closer to being alone while the only way to ease this paranoia is to take more pills which is why we see him take 2 pills at a time while thanos only takes 1
