I infodumped in my own replies but idk if ppl can see it so copy pasted it here: as it was explained to me in my anthropology class, most cultures have ideas of gender. you'll get some sort of split based on external anatomy and bodies, but cultures can add on one's for in between anatomy or those that switched from one to the other in their presentation (sometimes they're called different things, sometimes the same). but these cultures may not have the same ideas about what gender means. you may not have people who feel the need to transition if your culture just associates gender with bits (ie names for someone with one or the other). in cultures where there are more defined roles surrounding behavior and dress, you're more likely to see transitions.
