a lot of them you still get your period, now when you think of pregnancy the first thing that comes to mind is the monthly bleed. if you aren't showing and still bleeding youd assume you weren't pregnant

if you're not seeing a bump form at all but still getting sickly symptoms you'd most likely put it to something else first especially if you're not actively trying for a baby or even want one

I used to feel a knife cut feeling when i ovulated before I went on bc and horrible periods so not everyone would know right away. I didn’t get really bad pain until my appendix was trying to rupture which it did
