basically there was split realties so another version of them who went through the bad stuff those memories passed onto the sakuta at the end which is how he randomly remembered its bc an alternate version of him had those memories, that’s why sakuta says he dreamt of a girl (shoko) when they went on dates & mai had that strong desire to film that movie but it was bc another version of her had those memories which caused this new version of her to make that movie & raise awareness for organ donors which caused shoko to get surgery & that’s the reason she was able to live in the new timeline

Shoko survives because Mai starred in a film about Shokos heart condition and because of this there are more heart doners whilst in the original timeline where Mai didn’t play a role in that film it never released and made more people donate their hearts. Also at the end is when Sakuta remembers her and then I believe it’s mentioned in the post credits scene that Mai ends up remembering her a bit later
