If i were you id just ASK and be respectful. “Im noticing some inconsistencies in your growth journey, Are you taking any new meds or changed anything since we first began our journey?” Its perfectly fine to share your OWN medical records, its only wrong if someone ELSE discloses medical information thats not their own.
