I think it’s Autism. Noise overwhelm them so they cover their ears. My son is in the spectrum and he is so special.

The two people could suggest a dissociative disorder, like DID. But the grey colours imply something like depression. I don't think the whale will be random though, yet I can't think would it might symbolise. I'm not actually sure, I don't know
