Why don’t we use this Neo more in an ICU setting post intubation/RSI. Especially in hypertensive patients that are solely related to sedation and rsi meds?

interesting. Neo is our first line in my ICU. They dont even let us use levo on our post op hearts, only epi, dopamine, and dobutamine.

this is quite a bit of an oversimplification. You get quite a bit of distributive shock after bypass, which is why levo can be useful. The Beta activity is much lower than the alpha activity so the argument about the tubes being smaller doesn’t really change. If you need contractility you add dobutamine or epi. Some places use neo over levo even in the intraop period.

Great explanation. Our pressor of choice on CPB is neo, and since we’re on pump and often times the heart is arrested it’s perfect.

Some CV surgeons only use neo and refuse levo at all costs (CABG/Valves) it’s the least arrythmogenic and i saw no difference in their outcomes. Fantastic post op drug to use.
