The superior oblique muscle depresses and intorts the eye. When it’s weak, patients experience vertical diplopia, especially when looking down (like reading or going downstairs). To compensate, they tilt their head away from the affected eye to realign vision. That’s why the cashier unconsciously tilts his head to the right — it helps fuse the images. • CN III palsy → ptosis, “down & out” eye, possible dilated pupil • CN VI palsy → impaired abduction • CN VII palsy → facial droop • CN VIII palsy → vertigo/hearing loss ⸻ Answer: D) CN IV (trochlear nerve) ✅

It could also be VIII. If there was a vestibular loss that was partially compensated it could cause double vision. Turning better ear towards the speaker to compensate for hearing loss and trying to better face them to “lip read”.

A, C, and D can all have control over eye movement and visionary attributes, but I believe only the Abducens (CN VI) can control eye movements going left or right.
