The ‘trifecta’ refers to the well-documented co-occurrence of hypermobile Ehlers-Danlos syndrome (hEDS), Postural Orthostatic Tachycardia Syndrome (POTS), and Mast Cell Activation Syndrome (MCAS). Despite being classified as separate conditions, they cluster together at rates that have led researchers to investigate a shared underlying mechanism. In a 2015 study (Cheung & Vadas, Journal of Allergy and Clinical Immunology), 66% of patients with hEDS and POTS also met criteria for MCAS. Recent research, including a 2025 proteomics study (Griggs et al., ImmunoHorizons), suggests immune dysfunction may run alongside the connective tissue and autonomic features of this cluster, particularly in women. Important: this is emerging research. The trifecta is not a single recognized clinical diagnosis. The conditions are real, but the unified mechanism is still being investigated.
