Dr. P’s Three Secrets to Diagnosing a Meniscus Tear on Physical Exam: 1. Palpation of Bony Landmarks and Joint Lines: Start by finding the end of the femur (thigh bone) and top of the tibia (shin bone). Next, press directly along the medial (inner) and lateral (outer) joint lines—where the menisci sit. Tenderness on either line may indicate a tear. 2. The McMurray Test: Bend the knee and rotate it while applying a varus stress (for the medial side) or valgus stress (for the lateral side). A click, pop, or pain during this twisting motion suggests a meniscus tear—you’re essentially pinching the cartilage between the two bones. 3. Combine Clinical Findings: These tests are even more accurate when paired with your patient’s story—like swelling, catching, locking, or giving out. Always test both sides. Bonus Insight: Did you know that meniscus tears occur in up to 40–70% of ACL injuries? That’s why, when we perform a BEAR ACL surgery or any knee surgery, we’re aggressive about repairing the meniscus, if possible. This involves placing sutures with specialized anchors to preserve this critical shock absorber. Even more exciting?
