It’s bc of the agencies in charge of Starship safety was dissolved during Khan’s rule. That orange leader put puppet administration heads who had no experience whatsoever with safety. All nepotism!

Seatbelts? Starfleet says where they are going they don’t need seatbelts. Go ahead and expand sickbay to at least 5 full decks and increase the size of the medical staff by 1000%. They got this. While Starfleet sits comfortably on earth lol
