Patient is a 73-year-old female do not resuscitate past medical history of hypertension congestive heart failure. I forget what COPD stands for Gerd. Patient is alert oriented times four presenting with eight out of 10 meaning pain and right lower quadrant. Pain is three days of nausea vomiting patient is nothing by mouth at this time.
