I used to work at a carnival and I set up and ran a zipper for several years. This should not even be possible if the ride is set up correctly and inspected daily. The parts that would have to be severely worn or missing would be noticed long before something like this could happen if the operator and ride supervisor were doing their jobs and paying attention. There's no excuse for the level of negligence it would take for this to happen. And why is it that not a single one of the many news reports mentions the name of the ride company? It gives the whole industry a bad name...
