But here’s the paradox: • A catheter or pacemaker has physical, testable parameters. • A large language model (LLM) evolves, learns, and impacts care in ways far beyond traditional “devices.” ⚠️ Today, if you brought an LLM to the FDA and called it a medical device, there’s no real pathway. This creates a dual challenge: • For innovators → Unclear routes to validation & approval. • For regulators → Struggling to catch up with exponential AI progress. The question isn’t just compliance — it’s trust. Patients and physicians need confidence that these tools are safe, reliable, and beneficial. Without a pathway, that confidence is hard to scale. 💡 My take: We don’t need to fit AI into old frameworks.
