Alex couldn’t leave because he knew he, as a trans man, couldn’t be accepted in any other place with a baby. So he leaned into his assumed role of the “protector” because as Abbie said, he is a coward, he was scared of his real identity and therefore decided to maintain his nuclear family ideal. He was heavily manipulated and controlled by Laura and couldn’t escape from it at the end. It symbolises the continuous cycle of control and torture that happens in real life cults and toxic communities, which u can’t escape from.
