@Dr. Amy Killen MD To further add to your post and make sense of the study …The new study shows that blood estrogen levels from patches vary a lot between women, so the idea that more estrogen always means needing more progesterone isn’t always true. Women with a uterus still need progesterone to protect against estrogen risks, but the right amount depends on how their body responds, not just estrogen dose. This means hormone therapy should be personalized and guided by medical advice, not fixed rules.

This study actually shows so much more than just estradiol and progesterone ratio fallacy. 1 in 4 women on maximum licenced doses still have low estradiol (under 50pgml) and 1 in 6 on high off label doses had low estradiol. This symptoms only, transdermal only basic .025 .05 & .1 needs to be discontinued. It is not serving women well at all. There’s no mention of FSH which is a significant marker of adequate individual estradiol dose suppression between 5-10. Ultrasounds to assess lining at least yearly needs to be part of hrt care as commonly as mammograms and OBGYNs need to stop running completely unessasary biopsies on women causing fear and significant trauma because of the slightest bit of spotting post on hrt. I can’t tell you how many women have thin linings slight spotting and are told they must have a biopsy it could be cancer! It’s terribly painful for many and they’re so traumatized they want to stop their hrt and suffer with the devil they know. Women need higher doses, women need diversity of delivery for all their sex hormones (self injections are awesome) labs need to be assessed at least yearly and more frequently as the doses are being dialed in, shbg and fsh need to be included in serum testing and providers need significant training on how to order and interpret sex hormone serum labs. Hormones are fluctuating is a cop out. End rant 😬
