The waist is not as small as it looks. Yes they did some waist synching but to achieve the crazy hour glass look they would use things like hip paddings/bustles and the super sharp drop waist to crest the illusion of a very tiny waist.

It’s because she was a child and also bc of the way the clothes are structured makes the waist appear even smaller. Most of those exaggerated silhouettes were achieved with heavy padding and shape language to get the desired effect, tightlacing was uncommon and honestly made fun of by some

I think it’s for multiple reasons, obviously brides were often teenagers, in addition to limited access to food in comparison to today, so they probably often weighed less but also many were malnourished
