Patients just don’t get!! I’m a retired CDA of 40 years abd they act like they’re the ONLY patient that matters!! I’m so glad that I retired!!

If she had had dental work done. She would’ve had to fill out the patient information form. And they include your address. What is the issue. Paperwork is not available right after a appointment because chart needs to be updated and it’s not always practical to do it while the patient sitting in the chair
