I really think that Cas never really realized it, that's why he said "the one thing I want is the one thing I know I can't have". Also I am perfectly confident same goes for Dean,he never really realized how much Castiel loved him,that's partially why he was so shocked in his confession. This and the fact he didn't thought he deserved him, "don't do this Cas" wasn't Dean rejecting him, what he really meant was that he didn't want Cas to sacrifice himself for him, he didn't thought he deserved him or his love.

The fact that dean truly did do EVERYTHING he could to get Castiel out. He searched for months when he could’ve left, but he stayed to search for somebody who didn’t want to be found. Even when castiel was the one that left, he continued to blame himself.

I also noticed after watching it 3+ times, dean was rendered SPEECHLESS and unable to stand every time Castiel died. He didn’t NOT accept his confession, he COULDN’T. he was literally unable to talk or make sound, which even when it was silence for that good few minutes he didn’t make a sound until he started sobbing
