On the flip side, I was referred for surgery for an l5/s1 herniation with significant sciatica, and now that we get closer to the date these symptoms have mostly resolved, so I am left wondering if I should be doing the discectomy or if I should wait and see how long between recurrences I have now. If its a week or two of trouble every couple of years I can certainly live with that

Great explanation! With those Modic changes at the L5-S1 level basivertebral nerve ablation could be a great minimally invasive solution for the any residual back pain. Especially if the transforaminal improves the radicular symptoms. Excellent work!

discogram is poorly predictive, and anesthetic disc injection has little published evidence of diagnostic efficacy. there are modic changes present....her axial pain may respond to basivertebral nerve ablation, if it is typical anterior collum type pain. i agree that the snrb may help with planning of decompression for the leg symptoms.

I had L4 L5 out after braking them 20 yrs ago instant pain relief .I ve had sciatica for 4 months the pain has been horrendous I was desperate last week so I had acupuncture & it worked I had my second appointment today ,I’ve gone from 10 to zero pain I just hope that it will last

Where are you located? I’ve tried everything as well PT, injections, chiropractic. My MRI showed I have several disc bulges but was told just go back to PT or pain management. Can you help?

This is me ! But realistically the injections will wear off , and I am certain the NHS won’t be willing to keep injecting . Took me a year to convince the doctors I needed an MRI. So what are the alternatives after injections ?

I went to a back dr 6 years ago with awful back pain. he did shots in my back which didn't help. I couldn't sit down for 16 month. Couldn't even eat supper not one time with my family sitting down. Finally it got some what better. Now I'm dealing with this all over again. Having trouble walking, tripping over my own feet daily. I can't take much more of this. I'm in so much pain and no one will fix it

Follow the neurosurgeon! The measure of a highly competent surgeon is knowing when NOT to operate. Neuroscience informs on neurogenic pain -treating the human spine like car mechanics is not the remedy unless there is COMPELLING CLINICAL EVIDENCE or CES, cervical myelopathy etc. lumbar radiculopathy is often unchanged or symptoms are worse with outdated surgeries. Blew out L5 in my 20s. Have had numerous surgeries and last one 5 years ago where they cut me in the front and back and fused 3 levels. Took care of 85% of the bad pain. Now, I just get nerve blocks here and there. Lasts 4-6 months. Stay away from the knife of possible....
