I can understand not wanting a consult 15 minutes before your shift is done and am personally willing to wait if possible and call the next person, but not wanting a consult at the beginning of a shift is insane. You can’t have it both ways.

As a resident who gets psych consults from the ER, thanks for handling the psychotic patients, getting labs and sometimes imaging even with the disruptive behaviors. I literally don’t care if I’m just coming on or about to end my coverage, I have it so easy in comparison to you guys. Also psychosis outside a secure psych unit can be so chaotic and risky so I’m honestly relieved when you guys call because then we can get them going to a safer spot to receive care!

My biggest issue with ER docs is they want to get as fast as possible to the point where they can pawn the patient off to the specialist. Haphazard work up completed, no real differential even made. Just…” hey, they need to be admitted and have abnormal labs”. At least try to figure things out a touch.

I hate when spealists tell me to go to the ER. I have complex conditions that can lead to serious complications BUT they are weird and super niche. I get that spealists have full schedules and can’t order stat imaging but they should be able to! Most ER doctors/radlogists who have never looked for a certain hard to read thing before will miss the probelm anyways. If the only reason I am being sent to the er is to make sure some weird super rare thing isn’t the problem it is such a waste of resources, contributes to over crowding, is a waste of time for everyone involved and aggravates my symptoms. Also I hate that urgent cares(at least in my area) can’t give iv fluids as someone who sometimes needs them urgently in order to not end up being an emergency, It is so frustrating when the system fails everyone. I truly appreciate you ER doctors for being funneled large amounts of patients who wouldn’t need to clog up your er if the system wasn’t so broken.

You’re doing the right thing! We seem to forget- always what’s best for the patient not us. As a specialist I always appreciate being called once the work up is done regardless of where I am in MY shift 😝

Consults were the most difficult thing for me to master in residency. Academia has such a breadth of experts available. Sometimes it can breed an environment of “load the boat” and “dilute liability” which creates some unneeded calls. I’ve been a nocturnist in every practice setting since training. Making personal relationships with consultants has made it so much better. They’re part of our team. I do as much as I can for them at work. They’ve saved my ass multiple times. I’ve taken care of their bounce backs and complications. Once they start to realize that you’re competent and make their lives easier I see the dynamic shift. The call becomes a conversation with a friend (i.e. “hey I need your help”) not “I have a consult for you”.
