Surveillance footage from the lobby showed him stepping forward as the elevator doors opened, expecting it to stop level with the floor. Instead, the cabin suddenly dropped without warning, trapping him between the cabin and the shaft wall, in front of horrified residents. The impact was instantaneous. Emergency crews arrived within minutes, but Waisbren was pronounced d€ad at the scene. Witnesses said the elevator had been shaking and misaligned that morning, but residents were still using it. Later records revealed that the building had a long history of elevator complaints filed with the Department of Buildings, more than a dozen in the months leading up to the accident. Several of these complaints specifically mentioned abrupt drops and uneven leveling between floors. Inspectors confirmed that the elevator in question had failed a safety test in May of that same year and was cleared for temporary use only after repairs were carried out.
