I don’t see it as a cockpit failure, there was a problem with the vision both pilots because the runway 28 had to be used, and obviously this was not lit up, and the controller left early without knowing if the pilots received the message, Strange story 😞😞

Sole issue was that they continued after reaching minimum decent altitude without the required visual reference. By the way Non Precision approaches like VOR/DME have no DA/DH it’s MDA/MDH.
