@thesilentwitness1: On June 2, 2015, at the Alton Towers water park in Staffordshire, two trains on The Smiler roller coaster collided after one was mistakenly released while the other remained stationary on the tracks ahead. The impact occurred at approximately 2:09 PM, trapping sixteen passengers more than 7.6 meters high. Front-row passengers Vicky Balch and 17-year-old Leah Washington suffered severe leg injuries resulting in amputations, while three others were seriously injured with multiple fractures and injuries requiring complex surgery. Emergency crews worked for nearly five hours to rescue those trapped. Subsequent investigations revealed that the safety sensors correctly detected a stationary car, but the crew manually overrode the system, believing it to be a false alarm. The error was described as "human failure to follow procedure" rather than a technical failure. Merlin Entertainments, the operator of Alton Towers, has been fined £5 million for breaching health and safety laws. The attraction reopened in 2016 with new safety systems and protocols in place.
