@dr.deepesh1: A 24-year-old male presented with a history of progressive blurred vision in the right eye for one week, accompanied by malaise, low-grade fever, and cervical lymphadenopathy. On further evaluation she also have history of eating pork meat which was most of the time uncooked. Toxoplasmosis is an infection caused by the intracellular protozoan Toxoplasma gondii, which can affect both immunocompetent and immunocompromised individuals. In healthy adults, the infection is often mild or asymptomatic, presenting with low-grade fever, malaise, muscle aches, and painless cervical lymphadenopathy. The parasite can be acquired through ingestion of undercooked or raw meat, particularly pork, lamb, or venison, or via exposure to oocysts in contaminated soil, water, or cat feces. Ocular toxoplasmosis is the most common cause of posterior uveitis worldwide and typically manifests as necrotizing retinochoroiditis. The parasite forms cysts within retinal cells, which can remain latent for years and reactivate later. Reactivation leads to focal retinal necrosis, surrounding inflammation, and infiltration of inflammatory cells into the vitreous. This results in the classic fundoscopic finding described as a “headlight in the fog”, where a bright white lesion is seen through a hazy vitreous. Patients with ocular toxoplasmosis typically present with blurred vision, floaters, and sometimes mild eye pain or photophobia. The active retinal lesion initially appears as a yellow-white patch with fuzzy borders and vitreous haze; over 2–6 weeks, it heals into a pigmented chorioretinal scar.Complications include permanent visual field defects .Diagnosis is mainly clinical, supported by serology for Toxoplasma IgG and IgM or PCR in atypical cases. Consumption of undercooked pork is a well-established route for acquiring toxoplasmosis, as the parasite can form tissue cysts in pork muscle.
