lineman here. on our system we call that open wire. the top wire is the neutral. we are not worried about the insulation flaking off since we rely on spacing between conductors. those things on the wire are "secondary breakers" they dont allow electricity to pass thru. it's actually a way to split load on to a different transformer without having to dead end the existing wire.

POCO doesn't follow NEC standards, red tape may mean something completely different. Look up codes and standards for the local POCO.

Those are insulators cut into the open wire between the 2 transformer poles. Only on hot legs, shared primary/secondary neutral. Otherwise those transformers would be paralleled and 1 could go bad and you’d never know until the 2nd went bad.
