The mission, designated STS-51-L, was intended to deploy a communications satellite and conduct scientific experiments, including observations of Halley’s Comet. However, unusually cold weather on the morning of launch compromised the O-ring seals in the shuttle’s right solid rocket booster. These seals failed under pressure, allowing hot gases to escape and damage the external fuel tank. Within moments, the shuttle disintegrated at an altitude of about 46,000 feet, scattering debris into the Atlantic Ocean Everythin. The disaster shocked the nation. Millions of Americans, including schoolchildren, had watched the launch live, eager to see McAuliffe become the first teacher in space. In response, President Ronald Reagan appointed the Rogers Commission, which concluded that the O-ring failure, combined with NASA’s organizational and communication shortcomings, caused the accident Spaceline. The shuttle program was grounded for nearly three years while safety measures were overhauled. Challenger’s loss profoundly reshaped NASA’s approach to risk management, engineering oversight, and public accountability.
