30 year Pharmacy Technician here… 100% incorrect. Her pharmacy needs to dig out the original prescription from the start date and determine who made the mistake. Some doctors document the correct dose but the RX could have been written wrong. Without that initial prescription it’s hard to say. Also, a patient starting on such a high dose should have been caught by the pharmacist even if the doctor flubbed it. The initial pharmacist who did the verification is in fact wrong from the get go. That drug is often started at a very low dose and titrated up if needed. Contact the pharmacy manager and the local pharmacy authority.

I would say it's even above the pharmacists fault- at least in the US, pharmacists are constantly battling with corporate overlords who make them do more(flu shots, covid tests,RXs) with less staffing.

I, as a paramedic, have had many patients who had no idea how to take their medication. Doctors didn’t explain anything. The pharmacist didn’t. It was up to me to explain how to take their nitro, their inhalers, etc.

The pharmacist should always be double checking the medication as it is being dispensed. I have ALWAYS double checked my medication because I’m on a quite a few meds but I know what I need to be taking because I have a medical background.

As a prescriber, unfortunately I have made a mistake and thankfully the pharmacist was aware enough to question me and we made the correct adjustments. We are human, we do unfortunately make errors . It’s a check and balances situation. I’m not going to put blame, but I’m grateful it was found out. Being a healthcare provider is really tough. We truly only want the best for the people we take care. It’s a very difficult job to do.
