First, I know that hurt like a MF when it landed on his arm. His arm is broken or lacerated for sure. Second, always strap the transmission to the jack. I had a jack fail and dropped a 600lbs transmission on my back as I was walking under it, but because I had it double strapped it stayed with the jack and walked away with just a big bruise.

so from what I see the floor had been covered in oil at some point and wasn't properly cleaned, also having a smooth shop floor isn't always safe for for exactly this reason
