We covered this in session zero. My group voted - No resurrection. Revivify is the only available spell. We called it hardcore mode. They once ended a dungeon with 3 levels of exhaustion, no spell slots or potions left. Told me the stakes is what made it fun.

My group gets to play AT MOST once a month. But the conversation is usually: Death is a real thing that can happen. If I poorly design an encounter, I will adjust. If you make bad decisions or the dice aren’t in your favor….. time to make a new character.

...I'm taking notes. My first modified Strahd campaign had my players meet a Night Hag in disguise and 1 or 2 characters wanted to throw hands. They Barely talked their way out of it. Buuut now I'm thinking I should've let the Hag get mad and reveal herself. Then teach a lesson.

As a person who had a fear of failure, DnD taught me that no matter what failure is an option and sometimes it can make the situation funnier. Or I can strategize. Either way I've learned to embrace it. But yeah if they were warned it's on the Game master. However I would probably do something where the party is revived by a necromancer that they now owe a debt to or fight their way out of the underworld

I had that arc with a character of mine. a teammate was slain by a cult group led by a werewolf. my character was saved by others when he was inert (warforged) and his friends death remained his failure. until he trained and took the werewolf out, and wears the pelt as a cloak

i have a specific campaign written out where if the party gets tpk'd they come back later in the campaign with various undead templates as the big bads curse to remove the god of death causes random groups of undead to form

Two of three of my players were trying to fight the Baba Yaga AT LEVEL 1. Thankfully the third party member was able to talk everyone down or I would have allowed her to wipe them because they weren't heeding her warnings.

100% removed a player awhile back for always deciding to not participate in any scenario that could fail. and be would sit there and ask if he could do his own thing alongside the party. once even asked for a private session for what they were doing during the party was doing the main objective

My group seen that failure can have consequences that changes the world more drastically and in interesting ways than success. Like how they failed to save the goddess of magic, and a series of events led to Vecna rising as the new caretaker of the weave.
