We had a seasoned nurse push MiraLAX in the IV on 2 patients. When she was approached they asked what she mixed it white she said sink water. Both patients ended up in the ICU on CRRT

not as a medical personal but as a patient: long story short I sneezed and lost my vision entirely. my neurologist said I was having a panic attack and called psych, my psychiatrist immediately knew this wasn't a panic attack and a huge emergency and had me sent in for an emergency MRI. had a 1cm clot in my brain. if my psychiatrist hadn't intervened I would have died

A nurse was trying to give 10ml of insulin instead of units ! Lucky another nurse caught her and stopped her and reported it. Then the nurse denied it all. 😳

I’m a Respiratory Therapist, worked ER/Trauma. Watched and tried to stop a travel nurse from giving an entire bottle of Regular Insulin. I’m a Type 1. She said I know what I’m doing! I said, Dr. Hall, ask her how much insulin she just gave. He was livid. He made her sit beside and check blood sugar every 10 mins. The patient was post full blown code. She never worked our ER again. Ever!

Pharmacist at a hospital here. We had a nurse who was supposed to pull 5mg of labetalol IV push and give it to a patient whose heart rate was high in surgery. She overrode the Pyxis and gave 5 milliliters of lantus- 500 units of long acting insulin that is only supposed to be given subq. The patient lived but we were compounding concentrated dextrose bags for IV use for HOURS. She tried to blame pharmacy because they both start with L and they were in the same Pyxis (completely different drawer though) 🤦♀️

I was in the ICU last October for DKA. My 3rd day there a nurse tried to give me 5 mL’s of insulin through my port. Luckily I was alert enough to stop her and another nurse intervened. Suppose to be 5 UNITS not 5 mL’s 😳

I was the vet tech for an animal shelter, and one morning I guess I was really tired, because when I opened a med bottle to get a dose for a dog, I popped the pill in MY mouth and swallowed it. My coworker just stared at me dumbfounded and said “did you just take that?” I said “yup.” Luckily it was just an antibiotic.

My other half is allergic to ibuprofen. He hurt his knee and was swollen but can’t take ibuprofen (anti inflammatory) I was asking the PHARMACIST what he recommended was best for the situation. He said “tell him to get over his lil stomach upset and take it anyway” umm it doesn’t hurt his stomach… HE STOPS BREATHING.
