Looks like obstructive shock, in a patient with COPD should ask yourself about severe autopeep or tension PTX, if not then can move on to addressing the pHTN

start inotrope. PAOP wnl, r/o overloaded. severe pHTN w/ elevated CVP. card shock w/ comp elevated SVR. milrinone 1st choice d/t pHTN but hypoTN, dobutamine then (?)

Inotrope. Output low, mixed venous low, wedge isn’t bad for an intubated patient without knowing their settings so wouldn’t reach for fluids first. Preload appears adequate. May need diuretics.

Pressors, inotropes, diuretics, and increase PEEP. Also, new studies show increased FiO2 can increase atelectasis so if the increased PEEP increases the SpO2 decease the FiO2.
