Yeah, but if his hemoglobin was 16 and he lost half his blood wouldn’t that still making an 8 why would you transfuse at that point? so you still kind of need a baseline, right?

You could take the analogy even further with carbonation. A “flat” soda has a lower H2CO3/dl because it’s not pressurized, losing CO2. Damn, you could tie it into blood gases too🤯😂

That's the same for transfusions too!! Hgb isn't checked immediately after transfusions for the same reasons. Thank you for the great analogy!!👍

That’s why I believe having an IStat in the operating room is basically useless. Why? We have obvious signs way before we get a hemoglobin via an istat

As a EM doctor some of my frustrations are with OB, with women presenting in the middle of the night with sudden heavy bleeding , usually post miscarriage , very pale clammy, tachicardic, chux pad full of blood / clots with initial “normal “ H/H . “ sound like she is stable , her hemoglobin is normal , why are you calling me ?”

This is why many are against resuscitating bleeding with crystalloids. You just dilute Hgb, Plt, coags and cause more problems

Okay so when I was a very new nurse I had a patient with a significant nose bleed on thinners with a normal hgb, and when I asked why this was, the doctor said “because math,” which was not helpful lol. Your passion for teaching is amazing!!

This is also a really great thing to mention with why we see cyanosis in COPD patients and pediatrics differently than “normal” adults. It’s about the concentration of hemoglobin that’s been desaturated.

I had a massive haemorrhage post-tonsillectomy. Class three haemorrhage, almost class dead. Extremely lucky to survive. I got fluid resuscitated, however I did not receive any blood products until day 4 in the hospital, despite loosing approx 1.5L of blood as my haemoglobin was at 76 (not sure the correct units here for aus) and was also told as i was B+, they had to wait until I really needed it.. Day 4 dipping into the low 60's did I start to finally receive blood and irontransfusions! It was a long recovery. Thanks for your videos, I always learn so much xxxx
