Progesterone is one of the most misunderstood hormones I see in fertility. When a blood test shows low progesterone, the immediate conversation often becomes: how do we increase progesterone? But clinically, I’m much more interested in understanding why progesterone was low in the first place. Progesterone isn’t produced at high levels throughout your entire menstrual cycle. Its production increases significantly after ovulation. Once an ovarian follicle matures and releases an egg, the remaining follicle transforms into a temporary endocrine structure called the corpus luteum. The corpus luteum then produces progesterone during the luteal phase. This is why progesterone can tell us something about what happened earlier in the cycle. If progesterone is lower than expected, I want to know: Did ovulation actually occur? Was progesterone tested at the correct time relative to ovulation, rather than automatically on “day 21”? Was follicular development adequate before ovulation? Is PCOS affecting follicular development or ovulatory function? Could thyroid dysfunction or elevated prolactin be interfering with the hypothalamic pituitary ovarian axis? Is insulin resistance or metabolic dysfunction contributing? Are there nutritional, energy availability or other physiological factors affecting ovarian function? This is why I sometimes describe progesterone as a monthly report card for ovulation. The progesterone result may not be where the problem started. It may simply be giving us a clue about what happened upstream. And this distinction matters in fertility. Progesterone supplementation absolutely has an important place when clinically indicated. But replacing progesterone and investigating why endogenous progesterone production may be inadequate are two different questions. Your hormones don’t operate independently. Fertility involves communication between the brain, pituitary gland and ovaries, alongside thyroid, metabolic and other physiological influences. That is why good fertility investigation should go much further than looking at a single hormone result.
