LVADs are preload dependent and afterload sensitive. One of the best ways is of course making sure preload is optimal by giving enough fluids to decrease the risk of suctioning events. Afterload wise is best to restrict the amount of vasopressors that we can give so that the LVAD can fucntion correctly as well as keeping BP under control. Not to mention inotropes to help with contractility

diuresis if needed to reduce preload, inotropes to increase contractility, vasodilators to reduce afterload, optimizing lvad settings, and placement of rvad if needed. the lv will become underfilled if the speed at which it is being unloaded is faster than the rv can deliver causing the septum to shift to the left, worsening the rhf and causing low flow alarms on the lvad.
