I kinda just realized that the “Stewie’s accent” exploration in this episode is a metaphor for Seth using goofy voices as a workaround for his speech impediment. When he hosted SNL, his monologue was a song called “Voices in My Head” where he got to do all the big ones. Audience ate it up, and he only had to sing and do characters. No chance of slipping up on live tv. The voices are a distraction tool just as Stewie’s affectations distract from his nightmare family life.

I adored this episode. Even the horrifically dark ending. Part of me believes it was the writers desiring to silence some of the "Stewie not being evil anymore" complaints.
It's a rare moment where the show examines actual cruelty and evil in a way that's very real to life. It's such a raw showing of what true villainy can really look like rather than just cartoon violence.
