Shell shock is a term coined during World War I to describe the psychological trauma and physical symptoms such as tremors, blindness, paralysis, and anxiety, suffered by soldiers exposed to intense, prolonged bombardment. Initially thought to be a physical brain injury from blast pressure, it was later understood as a "war neurosis" or emotional collapse, now recognized as an early form of PTSD. Key Aspects of Shell Shock: Origin: Coined in 1915 by British psychologist Charles Myers to describe the unique mental disorders seen in trench warfare. Symptoms: Ranged from physical manifestations (involuntary tremors, convulsive attacks, dizziness, stammering) to psychological issues (extreme anxiety, depression, insomnia, loss of memory). Initial Misconception: Early in WWI, it was believed that the shockwaves from artillery caused microscopic damage to the brain. Evolution of Understanding: As cases occurred in soldiers far from actual explosions, doctors realized it was a psychological response to extreme stress, fear, and horror, rather than just physical injury. Treatment and Stigma: Many victims were often wrongly accused of cowardice or weakness. Treatments ranged from rest to harsh measures like electric shock therapy, often with the goal of returning the soldier to the front line as quickly as possible. Connection to Modern Medicine: Shell shock is now understood as a predecessor to the diagnosis of Post-Traumatic Stress Disorder (PTSD). Symptoms of Shell Shock: Physical: Shaking/tremors ("shell shake"), blindness, deafness, vertigo, and paralysis. Cognitive/Emotional: Amnesia, profound confusion, panic attacks, nightmares, and depression.By the end of WWI, it was clear that the unprecedented scale of industrialized slaughter was shattering the mental health of soldiers on a massive scale, leading to a shift in how military medicine viewed psychiatric trauma.
