As a lawyer, …this tracks. I usually skip Dr notes and go right to the nurses notes. Just so yall know, we see you.

I called a doctor and the PA multiple times as charge because a patient was exhibiting CLEAR signs of compartment syndrome. They told me there was no reason to believe that. The shift ended with a BEDSIDE fasciotomy.

The amount times I have documented “MD made aware, no new orders received “ is wild

I had a doctor literally tell me he didn’t want to consult ICU about my patient who’s BPs were 60/40 because “ICU doesn’t do shit anyway” then he called back cursing at me because I documented it in the chart. Needless to say I documented that as well.

Boss asked me to falsify a chart. I said no & had my job threatened. So I charted "At the direction of (boss' name)" then typed the conversation verbatim & made that person cosign the note. They never asked me to do it again.

💙💙💙💙 when I was in the hospital the traveling RN was charting out loud "MD removed himself twice from the chat about PT getting their meds. 5 hours with no meds and no response from MD." guess who got their meds 10 mins later 🤣

Helping a newer fellow nurse, Oncology Unit, with a neutropenic patient but that wasn’t the problem. Something DRASTIC was changing very quickly and neither of us could put our finger on it. Called the on call doc and got a NP that I wasn’t familiar with who WAS NOT LISTENING!!! I knew the pt needed a higher level of care immediately and snatched the phone from that poor nurse and told NP what we were going to do. End of the story….pt was moved, pt passed away 30 minutes later, doc arrived to ICU. Wife was present for all of it. I don’t think it was preventable but ALWAYS trust your gut instinct. Not really on topic bc I was not asked to change my charting nor would I ever comply with that kind of request. I have a lot of respect for floor nurses who take their job seriously.

I am sure it's not the case but in a perfect world i feel like any doctor that gets caught trying to redact things or lie on documentation should have their medical license revoked.

“ENT stated that he’s never seen anyone die of a nose bleed”. Nurse replied, “Well this may be your first” after Hemoglobin dropped from 12 to 7.

I saw a lawsuit where one of the exhibits was an NP note saying she firmly disagreed with the physicians recommendations and that they could cause serious harm or death. The patient died.

OMG my infant seized day one. Rhythmic, robotic movement on right side only. I clocked it. Nurse clocked it. Doctor: Let’s get him an X-ray to see if his clavicle is broken. Clavicle: fine. Neurologist: MRI stat. MRI: Ischemic Stroke. (My son is fine now) INFANTS ABSOLUTELY HAVE SEIZURES!

Doctors should be forced to be nurses for at least a year before they get their higher degree.
