I was always asked not to chart "it." As in my assessment findings! Due to findings created problems with admissions and at discharge. I was only to chart pts chief c/o. extra had to be "dealt with." First year of nursing, ED/ICU. I don't think I'll ever work for a hospital again.😳

On my old floor the manager used to tell us to chart CHG’s even if we didn’t actually give them or to chart them for a time we hadn’t given them yet (ex: chart them for 2000 when I wasn’t able to give them until 2200, etc.)

@The Legal NP👩🏼💻 how about when you consulted them on something in the ambulatory setting. For example whether we are able to perform wound care that is more complex at the clinic.
