In August 1997, Fine Air Flight 101 (a McDonnell Douglas DC-8 cargo plane) was taking off from Miami International Airport, fully loaded with nearly 90,000 pounds of denim fabric bound for the Dominican Republic. Just seconds after lifting off the runway, the aircraft's nose pitched up violently and uncontrollably. Despite the pilots desperately pushing their control columns fully forward to force the nose down, the massive jet entered a fatal aerodynamic stall. The aircraft plummeted from the sky, crashing flat into a busy roadway and a commercial business park just past the end of the runway. The impact and subsequent fire tragically killed the four crew members on board and one person on the ground in their car. The investigation uncovered a catastrophic breakdown in ground handling and cargo loading procedures. The contracted loading crew had placed the heaviest pallets of cargo far too far back in the airplane, severely shifting the aircraft's center of gravity dangerously out of safe limits. Compounding this fatal error, the pilots were handed completely incorrect weight and balance paperwork, meaning they had set their takeoff trim incorrectly. The moment the plane left the ground, its extreme tail-heavy condition made it physically impossible for the crew to keep the nose down.
