L-theanine is a naturally occurring amino acid found primarily in green and black tea. It is known for its calming effects on the brain without causing significant drowsiness. Researchers believe L-theanine may influence neurotransmitters such as GABA, dopamine, serotonin, and glutamate, which help regulate mood, attention, stress responses, and sleep. It may also increase alpha brain wave activity, which is associated with relaxed alertness. Because of these effects, scientists have been investigating whether L-theanine could help people with psychiatric conditions, including ADHD, schizophrenia, obsessive-compulsive disorder (OCD), depression, and anxiety. A 2024 systematic review published in BMC Psychiatry examined 11 randomized controlled trials involving 827 participants who received L-theanine across several mental health conditions. In patients with schizophrenia, 400 mg daily for 8 weeks, alongside antipsychotic medication, improved certain psychiatric symptoms and anxiety scores. In children with ADHD, 400 mg daily improved objectively measured sleep efficiency, while a separate small trial suggested that combining L-theanine with caffeine may improve aspects of attention. For OCD, 200 mg daily alongside fluvoxamine for 10 weeks produced a complete response rate of 92%, compared with 36% in the placebo group. Another trial found that adding 200 mg daily to sertraline for 6 weeks increased depression remission rates to 68%, compared with 32% receiving sertraline and placebo. However, results were not consistent across all conditions, including generalized anxiety disorder, and the researchers emphasized that larger clinical trials are needed to confirm these benefits.
