@im.nurse.liz: Do I like making people upset? Absolutely not. And for a long time, I messaged back and forth about everything. Until it took up all of my evenings. And hours out of my weekends. And I had to start implementing boundaries so I didn’t rage quit: again 🫠. I absolutely get it from a patient perspective that appointments are hard to get and make it to and they’re expensive. And with every ounce of me I wish it wasn’t that way. But doing work ups over messages, isn’t sustainable. It’s unpaid labor. And it will burn you out in no time. So boundary up. Prepare to get some heat, and repeat after me. “I want to give this the time it deserves, please schedule an appoijtment” and “I would love to, but unfortunately i can’t”. How do you handle this?? MyChart should have an automatic message that pops up after the 2nd reply back to patient that says if the first response wasn’t want you wanted to hear call the office and make an appt

Primary care PA here - the messages/my chart are the main reason I will probably be leaving PC. Which stinks bc it’s such an important field, it’s such a lost cause. If you don’t respond quick enough, you get a 0/10 review

Make a quick phrase. "I'd be happy to discuss this with you in clinic or via telehealth, please call and schedule at your convenience." And educate your staff to use it.

I forward to my MA or nurse and say “ needs to be seen” and keep it rolling. They will be able to explain this to the pt and get them scheduled. I give very little time and effort to things that I don’t have to. Too many other things to do.

I had a patient message in on Christmas Eve at 6:30pm (our office closed at noon) asking for antibiotics, and the. Got a second message two days later yelling at me because she ended up in the hospital with pneumonia. Like ma’am, expectations 🙄

I recently learned how to billfor these. If a patient initiates the message and you order anything from it and they’re not seen within that week you can bill an E visit.

Our office policy is as follows: “If you are wanting to speak with the provider and get any medical advice, an appointment is required for documentation purposes.” “Any new medications or doseage change requires a visit for medical advice and documentation”. My provider’s do not answer any messages via the portal. They are too busy seeing the patients in person.

as a MA,who thankfully the boss listens to me, I refuse to set up pt messaging. do I hate 20 voice mails every hour? yea. I really do. but pts messaging their provider is an absolute NO in my book.

How long is the wait to see you? I find myself in the conundrum often that I don’t have open appointments for weeks. Then on top of that, they outsourced our phones and it is like playing the telephone game so they do the MyChart messages to make sure that I get the message they want conveyed. COVID opened this Pandora’s box and it just won’t close. It is definitely going to be what will end me in the long run if I can’t figure it out I’m afraid

99421, 99422, and 99423. They aren’t a lot per time, but it adds up and makes it clear that your time isn’t free. Also helpful to track so that you can justify staff to assist

As an EHR analyst, I would highly recommend that you work with physician leadership to develop an organizational or practice policy about how to manage these. For example, “according to the policy, medication, lab, and radiology orders my accompany a visit (in person, virtual, or both); also I would say there should be a max number of messages that a provider can reply to per visit scheduled. So you can answer 2 messages after a visit. This will help implement something in the EHR to mirror that
