@ventedcrna: In the ICU, the loud problems get attention fast. It is the quiet changes that get missed. The patient who stops asking questions. The family member who suddenly goes silent. The subtle shift in breathing that does not trigger alarms. The calm that feels out of place for how sick they are. By the time the numbers change, the body has usually been compensating for a while. And when compensation runs out, it runs out fast. This is the part of critical care that does not announce itself. It whispers. And if you have been burned once, you never ignore it again. New nurses are trained to watch the monitor. Experienced nurses learn to watch everything else. Critical care nurses — what is the quietest sign you have learned means a patient is actually doing worse Drop it below 👇 These comments matter. Neuro ICU here. It is the quiet. Going down on sedation and ✨nothing✨ happens.

Sudden burst of energy. Eating when they haven’t in weeks. Tachycardic while unresponsive. Fever for no reason. Certain odor they put off. Mottling. Seeing deceased loved ones. Agitation. Restlessness. Increases confusion. Breathing changes.

I worked security in hospitals for 7 years. I’m not a nurse, and call me crazy if you want, but the smell of the room starts taking on the faint scent of death. It’s slight but it’s there.

It sounds weird, but your “gut feeling” hits when you walk into the room and they look “off” compared to prior assessments. Usually r/t their communication/arousability😅

Life support & ICU for 17 days. They told hubby multiple times I was on my way out. That was 5 months ago and I defied every one of them.

I’m a vet tech, so no disrespect, but in animals the fur will stand up on their whole back all the way down their tail. Poofy. Cats will purr when they’re really sick. We call it the death purr… they also usually rally right before. Like, suddenly want to eat or get up and move around. But the fur standing up is a sure sign..

The truly quietest sign is the patient who simply hasn’t really improved. Their pressor doses haven’t gotten any worse than they were 3 days ago, but they haven’t gotten better either. O2 reqs about the same, still delirious but not climbing up the walls, etc. The best predictor of a critically ill patient who will do well is one who gets better every day. A day in the ICU where nothing gets better means a day where the patient is doing worse
