I attended a pt with hereditary angioedema a handful of times, first time I encountered them they were unresponsive. I stopped freaking out when I realized they could still breathe through their nose. Also they had this really expensive IM med in their fridge that we would administer to them, Icatibant.

If you have a patient like those described, do you choose to not paralyze them, and just sedate only incase you’re unable to obtain, and they can still have their respiratory drive? Or what’s your practice for medication for first pass?

On a debrief after a call where I did a surgical cric, my MD said he would have backed me skipping an intubation attempt at all given the scenario. I said, "I know, it's just a hard cliff to jump from," and he said, "a patient can recover from cric a lot better than they can a hypoxic brain injury." That one epiphany took my hesitation away on future calls where a surgical airway is indicated.

RT here, even more last ditch effort is the needle crich with jet ventilation. I’ve seen it twice and buys time for a definitive airway.

Question! Why a burn? When you say burn are you meaning more topical or say the patient was in a burning building and we would be worried about soot? Or both maybe? (Premed student curiosity) 😊
