The reason why some complex on an ecg are “upside down” is because the vector of electrical activity in the heart is traveling away from the electrode of that specific lead. For example, AVR which is augmented vector right, is opposite of the normal vector of the average electrical activity of the heart which is down an to the left. When the vector of the heart travels away from an electrode the deflection of that lead is downwards. Read more about the hearts electrical axis to learn more about how axis deviations occur.

That looks like a 1st degree and a left bundle branch block. The QRS looks to be at least 3 or more boxes, so it would make it 0.12ms long, which falls under BBB criteria

it's not upside down per sé. A vector can change, also you don't know which lead you're looking at.
