Joey Nobody, R.T.(CT)(R) :
anyone doing IV access should really be shown how bad contrast infiltration can be

When I was a baby tech I was seconds away from using a Cordis for an angio. I finally turned around and told the nurse I gotta be honest, I’m not comfortable doing this. That line says nothing about pressure rating on it. He made a call and said don’t use it. Glad I trusted my gut!

So many variables, but don't come at me with an IV hanging half way out or a 22 in the hand and ask me to just try it, because it flushed well with a hand push. That IV won't hold up with contrast injected at 4cc/sec or higher. Trust your CT technologist's judgements.

Idk if you mentioned this in your previous videos but people NEED to know the difference between an infiltration and an extravasation. An ED nurse looked me in the face and told me there was no difference between a saline infiltration and a contrast extravasation. There is a difference and thought compartment syndrome is rare, an extravasation needs extra precautions

What is an appropriate reason to do a sinus with contrast exam?

Not normally I would agree about the 22 gauge IV, but if they are a really hard stick and they are super tiny and small, you can use a 22 gauge at 3 1/2 cc second. As long as it flushed as well by the way.

We (pediatric hospital) recently changed from using diffusics to B Braun - we can power inject a 24G CTA 22G and larger.

I like to tell the nurses, that our contrast is about as thick as d50. would you inject d50 at 4ml per second into that IV? at one point in time one of our nurses felt the contrast, she said it's about the same as the d50. That's where I got that association.

It’s crazy because I just had an issue related to this yesterday. Pe study and they wanted to use a hand iv. 3 ivs later and we were able to do the scan

Great Job!! You explained it very well!! Injectors will sense the back pressure of a small gauge catheter and my shut down mid injection. That is a safety feature within the injector. Not to be confused with something wrong with the injector itself go back in the history and see whether the injector overpressured.
