There are depictions of bad therapy and then there’s bad therapy-related dialogue. This is the latter. Don’t get me wrong, I think that this scene from Hannibal does what it needs to do (I’m not a screenwriter and really not one to judge from a craft perspective), but this line about “doctor patient confidentiality working both ways” is the sort of thing that probably sounds good in a writer’s room, but needs to be double-checked by an actual mental health worker (I’m open to script consultation if you want for this sort of thing).  The truth is, that the information that comes out in a therapy session is typically privileged and confidential. Moreover, recording therapy sessions as either the therapist (a common practice especially for training purposes) or as the client (rarer) has legal/ethical implications and should always be discussed/consented to on paper before it happens.
